New Patient Registration Process

  1. Complete the registration form, then “Continue to Additional Details page” button to add additional details.
  2. Enter your payment information. This will NOT be charged until your visit. HSA’s accepted.
  3. Scroll to the bottom of the page, click the checkbox, and click the blue “Next” button to move to the next page and complete step 4 on the next page.
