Concierge Medicine vs Direct Primary Care

There are 2 emerging trends in the US around primary care services: (1) concierge medicine and (2) direct primary care (DPC).

Current problems in primary care:

These two similar but uniquely different models of healthcare are trying to solve some common problems in our primary care system. Patients are seeking personalized healthcare with a physician that they know, like, and trust. Also, they would like to have access to that physician when they need it. 

Unfortunately, It is difficult to get a timely appointment in most primary care offices. On average it takes 26 days to get an appointment to see a doctor. This is due to a combination of a physician shortage and the need for physicians to see more patients (in short appointment slots) due to the decreasing payments from insurance. It is typically difficult to speak directly with your doctor outside of office visits due the large number of patients that they see on a daily basis. This leads to hurried doctor visits after long waits in the clinic and then being limited to 1-2 concerns during the visit so the doctor can rush to the next patient. 

Concierge medicine and DPC seek to solve these problems:

Direct primary care and concierge medicine both work to provide a higher level of service to their patients by having increased access and longer office visits. To make this model of personalized healthcare work they seek payment outside of the insurance system. 

In the concierge model, patients pay a membership fee to be part of the practice AND the practice also bills the patient’s insurance. Typically they commit to at least a year long membership. 

Direct primary care (DPC) does not accept insurance for office visits and instead has a low monthly membership fee that allows direct access to their physician through in person or video visits, text, email, or phone calls.

In addition, DPC practices typically offer discount pricing on labs, medications, and imaging.

Also, we contract directly with labcorp to get low cost labs and work with imaging centers to get the lowest available pricing.

New Family Medicine Clinic in Hendersonville, TN

How we got our name…

Blue Ocean Family Medicine was named after our first trip to Tennessee. Tara (my wife and now practice manager) and I had just put an offer on a house in Hendersonville that had been accepted. It was official we would be moving and opening a new family medicine clinic in Hendersonville. 

On a return flight to Idaho, we were discussing the business plans of opening a direct primary care practice and knew that one of the first steps was to choose a name. Tara was working on her homework for her MBA and was reading about blue ocean business strategies. 

A Blue Ocean Strategy

She described to me that a blue ocean strategy is choosing to create a business that is not in existence today or in an unknown market space. By creating something new you are in a market with no competition allowing for unlimited potential and growth. This type of strategy creates new demand for a product or service instead of being in direct competition with other businesses. 

This is opposed to a red ocean, or the market and businesses that are already in existence. It is called a red ocean because the competition is cut-throat leaving the ocean a bloody red. There are too many competing businesses looking for the same consumers. 

We decided that we wanted to open a family medicine clinic in Hendersonville using a blue ocean strategy…

and create a business that is truly unique and would allow us to really help people become and stay healthy. To do that we decided to open a direct primary care practice. By removing insurance middlemen, the physician-patient relationship is restored. Patients pay a membership fee directly to the practice and in return have direct access to Dr Lofley. Patients can now have TIME with their physician (30 and 60 min visits are standard) and easy ACCESS (in-person visits, video visits, call, text, or email and have same-day to next day access). In office dispensing of generic medications and at-cost labs provide additional savings to patients. This is our blue ocean strategy for primary care.

We now had the beginnings of a name for our practice. After several renditions we settled on Blue Ocean Family Medicine. We then held a design competition through 99designs.com where we outlined what we were looking for in a logo. After selecting our favorite designs we were able to further refine what we liked until we developed our current logo with a wave and a cross. 

Even better than the blue ocean strategy is the imagery surrounding a blue ocean.  When I close my eyes and think about a blue ocean I can hear the waves crashing on the beach, sense the sand between my toes, and feel the sun kiss my skin.

The environment is so healing

When you step into Blue Ocean Family Medicine we want you to feel a healing environment. There are beautiful photographs of the ocean adorning the walls (keep taking those beautiful photos Levon – saltycures.com). 

We have a Sisyphus Table in the waiting room that rolls a ball thru sand, all the time developing and erasing stunning patterns. Watching the Sisyphus is very calming.

6 ways to save money on medications

The cost of health insurance keeps increasing. Higher premiums. Increased co-pays. More out of pocket expenses. With all of these changes in healthcare, this typically means more out of pocket expenses — including the cost of medications.

I am always looking for ways to help patients save money on their medications. Here are 6 ways to help lower the cost of prescriptions.

1. Ask for generics.

Generic medications are made available after patents on medications have expired. These medications are made by a different company than the original manufacturer of the medication but are the same chemical compound. Using generics can help you save a lot of money, and they work well. Don’t be afraid to ask for generics at your next visit.

2. Price compare.

Medication cost can vary greatly depending on the pharmacy you use. I have seen the cost of a medication vary greatly. Today I found a medication that cost $212 at one pharmacy cost only $54 at another pharmacy. Also, just because your blood pressure medication is cheaper at one pharmacy doesn’t mean that they will have the best deal on your thyroid medication. I like to use the website GoodRX.com to do price comparison. In addition to finding the best price you can get prescription coupons. Which brings me to #3.

3. Use prescription coupons.

GoodRX.com has discount coupons for medications. You do need to know the specific name, strength, and quantity to get the correct coupon. This can be a huge savings. It isn’t uncommon to see a 70-80% savings. If you really want to save money then combine price comparison and use the coupon at the pharmacy that has the lowest price.

4. Get it through mail order.

As a way to save money, many insurance companies are encouraging their members to use mail order pharmacies. This saves the company money as they are working with a single pharmacy and they can contract for better rates due to a larger buying power. Some insurance plans will let you have a $0 copay for mail order pharmacies or a substantial discount. Check with your insurance company to see if they work with a mail order pharmacy.

5. Get a 90 day supply.

Most pharmacies will give you a discount by getting your medications for 90 days at a time. Although this may not be a huge savings it can really add up over a year. For example, if you take 3 blood pressure medications (generic cost is $4 per month per script) you could get a three month supply for $9 — a savings of 1 dollar per month. At 3 scripts a month for a year at regular price it would be $144 for the medications for a year. At a 90 day supply cost for 1 year it would cost $108 for a savings of $36 dollars per year.

6. Join a direct primary care clinic.

About Us

Dr. Kent J. Lofley, DO is board certified in Family Medicine by the ABFM and in Family Medicine and Osteopathic Manipulative Treatment (OMT) by the AOBFP. He completed a family medicine residency at Southern Colorado Family Medicine and upon graduation received the family medicine resident award for scholarship. He is a graduate from A.T. Still University in Kirksville, Missouri where he completed his Doctor of Osteopathic Medicine degree. He spent a year during medical school completing the Hollister Clinical Research Fellowship and completing a Master’s in Biomedical Science. Prior to medical school, he completed his undergraduate studies at Utah State University.

Dr. Lofley has practiced family medicine for 13 years. He has practiced in multiple settings including correctional medicine, outpatient family medicine, as director of a rural health clinic, and telemedicine. He served in the Air Force Reserves as a family physician and participated in a humanitarian mission called “Tropic Care” to care for indigent populations. He has also completed a medical mission to Guatemala.

Dr. Lofley has a special interest in OMT, in-office procedures, and preventative health. He is a lifelong student and loves to teach others. He currently sees patients of all ages and conditions with a special interest in helping patients reach their health goals. He opened Blue Ocean Family Medicine to help patients gain access to high quality, convenient, and affordable health care. That is why he chose a direct primary care model with a monthly membership fee. Dr. Lofley is the proud father of 9 children.


Tara Lofley, MBA is acting as CEO and manages the clinic. She graduated from Utah State University in 2023 with a Master of Business Administration degree. Tara dabbles in many things, but has an endearing love for listening to classical, symphonic and 80’s music, improvising in the kitchen, playing the piano, and singing. Among Tara’s most astute abilities are picking out a perfect watermelon, whistling, and always knowing where she parked. She teaches early morning seminary for The Church of Jesus Christ of Latter-day Saints, and has been married to Dr. Lofley for 22 years.


Alison Lofley, CPT graduated in 2023 from Nashville State Community College with an Associate in Science degree. She became certified as a phlebotomist technician from Northwestern Phlebotomy School in January of 2023. Alison is pursuing additional schooling with an interest in the medical laboratory sciences. She plays the piano, sings and loves to craft. Alison is the oldest child in the Lofley family.
